Today we show you a severely deformed case of rhinoplasty revision.
Patient had first rhinoplasty with silicone implant 45 years ago when she accidentally fell to floor. Afterwards she had 2 more revisions but since then she had redness and swelling symptoms. These symptoms evolved into infection which led to taking out the silicone implant. As a result she has gotten deformed and collapsed nose.

We can see the indent on her nose bridge. It almost looks like a scar but in fact it is uneven surface due to having partial implant on right side only and not having implant on the other side.

When seen from side, she had uneven nose line with very low bridge.
Red angle marked on the picture is nose-philtrum angle. When this angel is small, it makes your mouth looks protruded. She has small nose-philtrum angle.

Picture above is nose anatomy seen from side. You can see 'L' shape cartilage.
When operating rhinoplasty normally we leave 10~12mm of septal cartilage but in case of this patient she had only about 5mm. In normal nose on the left, cartilage bone supports its shape but in case of this patient she had only small amount of cartilage floating on the surface. Cartilage was moving around like log on a water pond and from the surface her nose bridge was moving around. She didn't have enough septal cartilage to perform revision rhinoplasty, we used donated rip cartilage and for the tip of the nose we used ear cartilage.

Before and after picture. Nose bridge is straightened and augmented. We also altered nose-philtrum angle to correct her protruded looking mouth.
